All Courses & Experiences require (at least) 3 forms to be completed - Medical Statment, Liability Release (either New Diver or Certified Diver or Enriched Air Nitrox Diver) and Standards of Safe Diving Practices.  These will need to be completed before any in-water training (pool or open water) can be conducted.

If Participant is under 15, their parent or guardian will need to complete & sign the Youth Responsibility & Risks form.

Can't Answer "NO" to all questions on the Medical Statement?  That doesn't mean you won't be able to dive but please take the complete form - with dive medical instructions - to your physician.  You'll need to have your doctor's permission before we can proceed with any in-water dive training.
